Why did I receive a W-9 form?

U.S. Bank sends notices to customers that have an invalid Social Security number (SSN), or Tax Identification number (TIN) associated with their account. These notices are sent following Internal Revenue Service (IRS) regulations. 
 
If you've received this notice, it means we don't have your SSN or TIN on file, or the number supplied was invalid according to the IRS and Social Security Administration. 
 
As this is an official letter, be sure to follow the instructions. If you don't, this could lead to added interest withholding and issues with the IRS. 
 
Here's a few other things to note: 

  • The notice includes a W-9 form and a return envelope to U.S. Bank's Legal Research department.  
  • We ask you to complete the form and provide your SSN or TIN so we can update your account with the correct information. 
  • The W-9 form can also be completed at any U.S. Bank branch.
