Did you receive a Reliacard® but did not apply for unemployment?

If you received a U.S. Bank Reliacard but did not file an unemployment claim, follow the steps listed below:

  1. Fill out the form and follow the instructions provided.
  2. We also encourage you to contact your local State unemployment office to report the fraud issue.
  3. You can also contact the three consumer credit bureaus to place a fraud alert.
