We suggest reviewing the document for the following information:
- Your first and last name
- Property address
- Policy number
- Loan number (can be hand written)
- Premium amount
- Policy coverages
- Effective and expiration dates
- Any policy exclusion(s)
If everything is in order, follow these steps to electronically submit your information.
- Visit ihaveinsurance.com/usbank and select Upload my insurance document.
- Provide your first and last name, loan number, and email address.
- Choose Select a document to Upload.
- Choose your insurance document from your saved files. The form will automatically submit your personal information and document.
- The option to use this service requires the insurance document to be in .TIF, .JPG or .PDF formats (less than 10Mb).
- ihaveinsurance.com/usbank is mobile friendly so you can complete this whole process directly from your smartphone and use your camera to take a picture of your Declaration Page.
