Loss Claim Process Instructions (PDF)

Contractors Statement (PDF)
This form is completed by the contractor and signed by both borrower and contractor.

Lien Release (PDF)
This form is completed by the contractor when repairs are finished.

Statement of Completion (PDF)
This form is completed by the borrower to confirm the repair work is finished.

Declaration of Intent to Complete Repairs (PDF)
This form is completed by the borrower to confirm their intent to repair the property.
