1. Actual individual savings and benefits may vary depending on the products used and balances maintained. Products and services for each Five Star Package have been combined to offer consumers preferred rates and services at a lower combined price than if each product or service were opened separately. For example, a customer in a Five Star Gold Package must have a $40,000 loan and direct deposit to their checking account to achieve $250 in annual savings.
     
   NOTE: A Five Star Checking account and Five Star Money Market Savings and/or qualifying U.S. Bank Credit Card must be opened with the same accountholder names on each account to be eligible for a Five Star Personal Banking Package and its benefits.

4. Reward bonus applies to U.S. Bank FlexPerks Cash Bonus Visa Check Cards and U.S. Bank FlexPerks Select Rewards, Travel Rewards and Cash Rewards Visa Credit Cards only. Earn a monthly bonus on the reward you earn during the month. Only signature-based purchases qualify for rewards. Cash advances, ATM transactions, and purchases made with a PIN do not qualify. The Interest Rebate is available on the non-rewards U.S. Bank Visa Platinum credit card and paid annually in November. The interest finance charge rebate amount is based on the total interest that was accrued in the previous 12 months.

5. Automatic investment transfers from Five Star Gold Checking to a U.S. Bancorp Investment account must be at minimum, a monthly transfer.

To continue eligibility for the Five Star Package benefits, the qualifying accounts must be open, active and in good standing. Qualifying accounts include the Five Star Checking and Five Star Money Market Savings and/or any U.S. Bank Credit Card.
